Field-based Clinical Pharmacist

Evolve are partnering with a leading pharmacy chain to recruit a Field-based Clinical Pharmacist. This is a great opportunity to provide expert pharmaceutical care across mental health and care settings, conduct prescription reviews, and deliver impactful training to multidisciplinary teams. Ideal for a proactive pharmacist seeking a varied, autonomous role with real clinical impact.

This is a permanent position offering both full-time and part-time opportunities, working Monday to Friday. The role involves supporting hospitals across the North West of the UK, with occasional travel to North Wales, providing a varied and engaging work environment.

Ideal Requirements for the Field-based Clinical Pharmacist
Bring your clinical expertise to life with at least 2 years’ post-registration experience in community or hospital pharmacy, making a real difference in patient care. 
Make an impact in mental health, combining strong clinical pharmacy skills with a genuine passion for improving outcomes in this rewarding and growing field. 
Enjoy autonomy and variety in your role, using your initiative to work independently while shaping high-quality pharmacy services. 
Advance your career with development opportunities, including support for further training in clinical or mental health pharmacy, with Independent Prescriber status highly valued.

Role Responsibilities for the Field-based Clinical Pharmacist
Provide expert clinical guidance and one-to-one support, empowering clinicians and nurses to deliver safe, effective, and patient-centred care. 
Lead on medicines safety and optimisation, reviewing prescription charts and driving interventions that improve patient outcomes. 
Champion high standards in medicines management, ensuring safe storage, accurate record-keeping, and full compliance across all settings. 
Design and deliver engaging training programmes, equipping healthcare and social care teams with the knowledge and confidence to manage medicines safely.
